Permission to Use Capstone Lab Form

I, ______________________________, agree to abide by the following rules
in order to use the Capstone Lab, Room 518 Cullimore Hall.

Rules:

I will use the Capstone Lab for course (Math 450H) related activities only.

I will not bring food or drink into the Capstone Lab.

I will not share the code to the Capstone Lab with anyone.

I will take good care of the equipment and keep the room neat, placing
all equipment in its proper place.

My use of the Capstone Lab will immediately end once I withdraw from the 
course or complete the course.

I will adhere to any other rules imposed by the professor or Department 
of Mathematical Sciences during the 2005-6 school year.
