1. Create a Microsoft Word document with all of the following information:

· Your full name

· Your e-mail address

· Your NJIT UCID (typically your initials followed by some digits)

· A statement saying that you will obey the University Policy on Academic Integrity.

· A statement saying that you have read and understand the syllabus. 

· A statement saying that you understand all the course policies (e.g., regarding late assignments, grading curve, no extra credit, minimally-working programs, requests for regrades), as described in the syllabus.

· A statement saying that you understand exams will be given online through Canvas and ProctorU Record+, that you have set up a ProctorU account, and that you have confirmed that your account is properly working through https://test-it-out.proctoru.com/

2. Upload the document in Canvas by the due date given in the syllabus. Late submissions will receive no credit.
