Please fill out the information requested below as soon as you encounter malfunctioning software/hardware on our UNIX/AFS systems. You can also use this form to submit UNIX/AFS-related hardware/software questions. The form is set to automatically send your name, UCID, e-mail, and the description of the problem to the AFS systems administrators. If your hardware/software problem involves Windoze PCs (either in the 6th floor lab, or in your office) please call the Help Desk at Extension 2900 to open a case. Don't forget to keep a record of the case number assigned by the Help Desk operator who took your call.
